Trimethaphan
Ganglionic blockade, now largely of historical interest, is the mechanism of trimethaphan: the agent competitively antagonises nicotinic acetylcholine receptors at both sympathetic and parasympathetic ganglia. Interrupting autonomic outflow at the ganglion produces rapid and controllable hypotension, and the effect ends quickly once infusion stops. Those properties suited it to deliberate hypotension during surgery and to hypertensive emergencies, aortic dissection in particular, where blood pressure had to be lowered precisely and reversibly by intravenous infusion. Blocking both autonomic divisions simultaneously also produces the parasympathetic effects characteristic of the class. The compound is recorded as C22H25N2OS with a mass near 365.5 g/mol.
